Femur is the longest, strongest and heaviest bone in the body. The femur can take a lot of stress and loads including axial loading, bending, and torsional forces. The neck of femur is designed in such a way to effectively transmit the weight of the body. The neck of femur has a good trabecular system. It is able to take a great amount of torsion and stress, but abnormally high levels of torsion or direct injury can lead to their breakdown leading to fractures. Mechanism of injury: Usually the neck of femur fractures may result either from rotation violence at the hip due to tripping over something on the floor and falling. This causes a direct violence over the lateral aspect of the hip and can lead to fracture of the neck of femur. This can occur in all ages, but is more prevalent in older people. Conservative management for neck of femur fractures: Continuous skeletal or skin traction is the treatment of choice if conservative management is necessary. The splint used to immobilize the leg is a Bohler Braun splint for those with coax vara. The foot end of the bed is also elevated. Traction force is applied appropriately with the orthopaedicians advice. In patients with no marked coax vara, skin traction using a Thomas splint will be enough. The period of immobilization is 12 weeks. Counter traction is got through the elevation of the foot end of the bed. Surgery for neck of femur fractures: The most common surgery that is performed in older people is the removal of the head of femur and replacing it with a metal prosthesis. The advantage of this surgery is that it helps in early ambulation and the patient weight bears earlier. Dynamic Hip Screw is the method of treatment for all other age groups. Open reduction and internal Fixation: Open reduction and internal Fixation or ORIF as it is commonly called is the treatment of choice for people with neck of femur fractures. There are many advantages of Open reduction and internal Fixation. They are listed below: 1. Early mobilization: The person can walk in a very short period after the surgery. Early mobilization after the neck of femur fracture helps in preventing bed rest complications. The bed rest complications that are prevented through Open reduction and internal Fixation are reduced incidence of pressure sores. 2. Muscle strength: There is also increased muscle strength due to early mobilization of the patient. 3. Reduced infection: Open reduction and internal Fixation does not have a high rate of infection like many other surgeries. 4. Weight bearing: Even after early mobilization, weight bearing may not be allowed in some surgeries. In patients with Open reduction and internal Fixation, early weight bearing is allowed. 5. Early functional activity: Open reduction and internal Fixation helps the patient with neck of femur fracture to return to activity very early when compared to other conservative managements. These are the many advantages of Open reduction and internal Fixation in a fracture of the neck of femur.
