"I get tired very easily",was the only complaint of this otherwise completely healthy young boy. Hypertension or high blood pressure is one of the far fetched diagnosis a physician would make for a patient his age and for the lack of other typical symptoms. With the stressful life each one of us is exposed to in todays world, we must all go for an occasional check up of blood pressure to avoid its deadly aftermaths. WHAT DOES IT ACTUALLY MEAN? Elevated arterial pressure normal limit is 120/80.where 120 is "systolic" and 80 "diastolic". A variation of 10-20 may be permissible in each but more limits can constitute Hypertension (HT). COMMON CAUSES OF HIGH BLOOD PRESSURE 1.Heart problems. 2.Thyroid. 3.Fever. 4.Kidney problems. 5.Endocrine problems. 6.Pre-menstrual syndrome. 7.Porphyria. 8.Birth control pills. 9.Hydrocephalus 10.Excessive blood transfusion. 11.High calcium in blood. 12 Pregnancy 13 Unknown causes. FACTORS MODIFYING THE COURSE - age - sex - race - smoking - serum cholesterol - obesity - glucose intolerance - existence of co morbid conditions like diabetes - working conditions and sedentary habits - dietary habits SOME EFFECTS OF HYPERTENSION 1. Effects on heart-inc in heart size, heart failure, right ventricle hypertrophy and angina. 2. Effects on brain-stroke, retinal changes, brain hemorrhage, occipital headache, dizziness, vertigo, tinnitus or tingling in ears, dimmed vision, syncope and encephalopathy. 3. Effects on kidney-leakage of proteins n blood in urine and kidney dysfunction, b>SIGNS AND SYMPTOMS OF HIGH BP AND HYPERTENSION Headache, dizziness, vertigo, palpitations, easy fatigability, impotency, epistaxis or bleeding from nose, hematuria or blood in urine, blurring of vision, angina and breathlessness. WHAT TO DO? - Give related family history to the doctor - History of any other disorder like diabetes,asthma. - LAB TESTS for HT * urine for protein,blood,glucose * hematocrit * serum potassium * serum creatinine or BUN * electrocardiogram * glucose,cholesterol and triglycerides * serum calcium,phosphate,and uric acid * chest x ray TREATMENT AND GENERAL MEASURES 1. Relief of stress 2. Diet modification-caloric reduction,salt restriction 3. Regular exercise 4. Control of other risk factors 5. No smoking 6. No alcohol 7. Decrease cholesterol n saturated fats,consume pufas. 8. DRUGS-diuretics, antiadrenergic agents,ACE inhibitors, vasodilators,calcium channel antagonists.
